Notice of Privacy Practices
How we protect your medical information.
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our duty
The Dot Theory is required by law to keep your health information private, to give you this notice of our legal duties and privacy practices, and to follow the notice currently in effect. We will tell you if a breach affects your information.
How we use and share your information without asking you first
- Treatment: with your nurse, our Medical Director, and the licensed provider who performs your Good Faith Exam by telehealth.
- Payment: to process membership and treatment payments through our payment processor.
- Operations: to run the studio safely, including quality review, training, and secure record keeping by service providers who are bound to protect it.
- When the law requires or allows it: for public health and safety, health oversight, lawsuits and legal orders, and law enforcement where required. Also to prevent a serious threat to health or safety, and to coroners, or as otherwise required by law.
- Care messages: appointment reminders, pre-care and aftercare by text, email or phone.
When we need your written permission
We need it for marketing, including any public use of your photos, and for any use not described in this notice. We never sell your information. You can cancel a permission at any time, except where we have already acted on it.
Your rights
You can:
- see or get a copy of your records;
- ask us to correct them;
- get a list of certain disclosures we have made;
- ask us to limit what we use or share. We will agree when you have paid in full out of pocket and ask us not to share with a health plan;
- ask us to contact you a certain way or at a certain place;
- get a paper copy of this notice.

Complaints
If you believe your privacy rights have been violated, email hello@thedtstudios.com. You can also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will never retaliate against you for filing a complaint.
Changes
We may change this notice, and the new notice will apply to all information we hold. The current version is always on this page and available at the studio.
